The obvious answer is that you already know the answer: he's out because the inning says it's time to take him out and no manager wants to be second-guessed on that move.

I suspect if you were to ask a manager why he'd give you an answer along the lines of not wanting to allow three tough hitters like Votto, Frazier, & Bruce to get swings against a 4th look at the same pitcher when just one swing can kill the lead. Whether you choose to believe that or not is certainly up to you.
