Teams need to rethink how pitchers are handled since Tommy John is a given nowadays. How many extra ligaments (technically I'm pretty sure they pull a vestigial tendon from your forearm, actually) do these people have?

Generally, the palmaris longus ligament of the forearm is used. For pitchers that have undergone numerous TJ surgeries or do not have a suitable palmaris longus ligament available, the surgeon sometimes uses a tendon from the knee or even "donor" tendon from the recently deceased.
