How many extra ligaments (technically I'm pretty sure they pull a vestigial tendon from your forearm, actually) do these people have?

For pitchers that have undergone numerous TJ surgeries or do not have a suitable palmaris longus ligament available, the surgeon sometimes uses a tendon from the knee or even "donor" tendon from the recently deceased.
